Future Stars Girl's Hockey Camp

This release of Liability and Indemnification agreement is made by and between Hockey4All, the customers whose name is subscribed hereto and the parent or legal guardian of any minor customer, hereinafter collectively referred to as the Parties.

WHEREAS, the undersigned parent of legal guardian of any minor customer has agreed and consented to their child’s participation and to the terms of this Release of Liability and Indemnification Agreement.

NOW THEREFOR, in consideration of the normal charge for participation in the hockey camp and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties contract and agree as follows:

Due to the inherent risk associated with the Hockey4All's Future Stars camp to include ice skating, ice hockey, recreation time, and off ice training including but not limited to: The ice surface itself, skate blades, hockey sticks, flying pucks, physical contact with other players, dasher boards and other inanimate objects, the operations of the zamboni, other ice surface equipment, wet slippery floors, outdoor recreation activities and physical training sessions, the undersigned for themselves, their children, heirs, executors, administration, assigns, guests and invitees do hereby waive death, personal injury, and damage resulting from the use of the ice rink and participation in the hockey academy, specifically releases Hockey4All, Owners and their subsidiaries, affiliates, directors, officers, instructors, and agents from neglect acts and omissions in the design ,construction, operating supervision or maintenance the Hockey4All and organization conduct of ice hockey sessions, practices, hockey, outdoor recreation activities and physical training sessions.

* No refunds within 30 days of the camp. Final payments will be charged 30 days out.
