This Agreement is between Moose Bear + Co. ("Provider") and the undersigned ("Client").

Client acknowledges permanent jewelry welding involves risks including but not limited to:

• Burns or skin irritation
• Allergic reactions
• Swelling or circulation issues
• Jewelry may require cutting for removal
• Loss or damage to jewelry

Client voluntarily assumes all risks associated with these services.

Client releases Moose Bear + Co., its owners, employees, and agents from all claims to the fullest extent permitted by law.

Client agrees to indemnify and hold Moose Bear + Co. harmless from any claims or expenses.

All services are final. No refunds.

This agreement is governed by applicable state law.

This Agreement is between Moose Bear + Co. ("Provider") and the undersigned legal parent or guardian ("Guardian") of the minor ("Minor").

Guardian consents to Minor receiving permanent jewelry services and acknowledges risks including:

• Burns or skin irritation
• Allergic reactions
• Swelling or circulation issues
• Jewelry may require cutting for removal

Guardian voluntarily assumes all risks on behalf of the Minor.

Guardian releases Moose Bear + Co. from all claims related to Minor’s participation.

Guardian agrees to indemnify and hold Moose Bear + Co. harmless from any claims or expenses.

All services are final. No refunds.
