Pre‑Boarding Questionnaire

General Boarding Information

Check‑In Times

  • Standard Check‑In: 3:00 PM – 6:00 PM

  • Early Check‑In: Additional fee may apply depending on availability.

Check‑Out Times

  • Standard Check‑Out: 8:00 AM – 12:00 PM

  • Late Check‑Out (after 12 PM): Additional fee or extra day charge.

Deposits & Reservations

  • Holiday/peak bookings may require a non‑refundable deposit.

  • Reservations are confirmed only once deposit (if required) is paid.

Required at Check‑In

  • Vaccination records

  • Signed Boarding Agreement

  • Completed Questionnaire

  • Medications labeled properly

  • Food clearly labeled with instructions

Overnight Boarding Terms

  1. All pets must be current on required vaccinations (Rabies, Distemper/Parvo, Bordetella). Proof must be provided before boarding.

  2. Pets must arrive in good health. Any pre‑existing medical conditions must be disclosed.

  3. Owner agrees that The Proper Pointer Co. is not liable for illness, injury, or death unless caused by negligence.

  4. Owner consents that their pet may be housed near or around other dogs.

  5. Personal items may be left but are not guaranteed against damage or loss.

Feeding & Care

  1. Owner will provide food OR agrees to pay for house food (Purina Pro Plan) if needed.

  2. Pets will be fed according to owner instructions documented in the Pre‑Boarding Questionnaire.

  3. Daily exercise, potty breaks, and fresh water are included.

Medication

  1. All medications must be labeled with pet name, dosage, and schedule.

  2. Medication administration fees may apply.

Behavioral Expectations

  1. Owner must disclose any aggression, anxieties, bite history, or special handling needs.

  2. If a pet becomes dangerous or unmanageable, The Proper Pointer Co. may modify care or require early pickup.

Payment & Fees

  1. Payment is due at check‑out unless otherwise agreed.

  2. Additional fees may include: medication, food, extra pets, special care, peak/holiday rates, late pickup, or damages.

  3. Deposits (if applicable) are non‑refundable inside the cancellation window.

Emergency Medical Care

Owner authorizes The Proper Pointer Co. to seek emergency veterinary care if needed (see full Emergency Authorization section below). Owner is responsible for all costs.

Emergency Authorization

  1. If my pet becomes ill or injured, I authorize The Proper Pointer Co. to seek medical treatment from Martin Veterinary Services or nearest available veterinarian.

  2. I understand that veterinary expenses are my full financial responsibility.

  3. The Proper Pointer Co. may transport my pet for care if needed.

  4. I authorize the vet to treat my pet as deemed necessary in an emergency.
