Invention Disclosure Form

Please fill out the form below so we have preliminary information about the invention. We will have an invention disclosure meeting (typically 30-60 minutes) to get full details of the invention. If there are no diagrams, figures, or flowcharts describing the invention, please consider creating some before the invention disclosure meeting. However, it is expected that during that meeting, we will create patent-appropriate figures and diagrams, but having diagrams beforehand is often helpful.
