Henna Brows: Ingredient Allergies *

I am not, to my knowledge, allergic to any of the ingredients in Lu Brandão henna.

Henna Brows: Precautions *

I understand and acknowledge that I should avoid henna if I have G6PD deficiency, have been advised to avoid fava beans or quinine, am extremely anemic, or have serious citrus allergies or essential oil allergies.

Henna Brows: Temporary Skin Stain *

I realize that my henna brow procedure is temporary and will potentially stain my skin for days, and will start to fade as soon as 24 hours after the procedure is complete.

Henna Brows: Brow Hair Tint *

I realize that my henna brow procedure will be tinting my eyebrow hair for up to 4 weeks.

Henna Brows: Results Vary *

I understand that results vary with everyone. Overall skin conditions and aftercare will ultimately affect the retention of the Lu Brandão results.

Henna Brows: Limitation of Liability and Arbitration *

I agree that the liability of LaJoy Grant and LaBrows Beauty Studio is limited to the cost of the procedure performed. In the event of disputes that cannot be amicably resolved, LaJoy Grant with LaBrows Beauty Studio and the client agree to binding arbitration to resolve any disputes, at the client's expense.

Henna Brows: Patch Test and Allergy Release *

I understand I can request a patch test to check for a potential allergic reaction to the henna products. The pigments used have a low probability of allergic reaction, but the possibility of an allergic reaction cannot be absolutely ruled out. The value of a patch test has been discussed with me. Whether or not I request a patch test, I hold LaJoy Grant with LaBrows Beauty Studio harmless and absolve them from all liability resulting from allergic reactions to henna products.

Henna Brows: Sanitation and Infection Risk *

LaJoy Grant with LaBrows Beauty Studio only uses sterilized, disposable equipment to minimize the risk of infection or contamination, and LaJoy has received training in appropriate sanitation and hygiene techniques. While the risk of infection is extremely small, it cannot be totally ruled out. I understand and accept this risk and release LaJoy Grant with LaBrows Beauty Studio from any and all liability related to this procedure, except in instances involving gross negligence.

Henna Brows: Form Explained and Refunds *
