I am the parent/guardian of the child(ren) listed above and give consent for them to attend and take part in the AMA Halloween Party (including martial arts games and activities) at AMA Marrickville on Saturday 31 October 2026. I understand martial arts activities carry some risk of injury and accept that risk. In an emergency, I authorise AMA staff to arrange first aid and medical treatment, including ambulance transport, at my expense. I confirm I have listed any allergies or medical conditions, and I will drop off and pick up my child(ren) on time.
