I voluntarily assume all risks, known and unknown.

I release and waive all claims against SoHi RevIVe PLLC, its owners, providers, staff, contractors, pharmacies, and affiliates to the fullest extent permitted by Florida law.

I understand this waiver includes claims based on negligence (excluding gross negligence or intentional misconduct).

I agree to indemnify and hold harmless SoHi RevIVe PLLC from any claims arising from my misuse or failure to follow instructions.

I understand I am waiving legal rights, including the right to sue.

I confirm I am 18 years of age or older.

I agree that any dispute, claim, or controversy arising out of or relating to my services, purchases, or this agreement shall be resolved exclusively through binding arbitration, not in court.
