Thank you for choosing MG Coaching for your virtual tax preparation needs. To ensure full IRS due diligence compliance (Form 8867) and accurate preparation of your tax return, please complete all sections thoroughly.

For our returning Clients, while we may have some this information from past engagements, we kindly request you provide your information again so we can make sure our records are 100% accurate and up to date.

Section 1: Tax Payer Information

Section 2: Filing Status

Just because you checked to file under a certain filling status, it does not mean that will be your filing status, as you might not qualify for it. Even if you have been filing this way previously. Your filing status is determined by what you qualify for. (Having dependents does NOT automatically qualify you as HOH).
By answering "Yes" you are confirming that if requested by the IRS, you can provide documentation that verifies you paid for over half the cost of keeping up your home, such as rent, mortgage interest, utilities, property taxes, insurance, food consumed in the home. If you use payments you received under a public assistance program to pay part of the cost of keeping up your home, you CANNOT count them as money you paid. 

Section 3: Spouse Information (if filing jointly)

By answering "Yes" you are confirming that if requested by the IRS, you can provide documentation that verifies your spouse did not live with you during the last 6 months of the year such as lease agreement, utility bills, or letter from social services.

A Qualifying Dependent must be blood related and/or be one of the following:

  • Son or Daughter

  • Brother or Sister

  • Grandchild

  • Niece or Nephew

  • Stepchild

  • Half Brother or Half Sister

  • Stepbrother or Stepsister

  • Adopted Child

  • Foster Child

  • Child placed with you for adoption

IRS Form 8332, the "Release/Revocation of Release of Claim to Exemption for Child by Custodial Parent," is used by a custodial parent to release their right to claim a child as a dependent to the noncustodial parent, or to revoke a previous release. The noncustodial parent must attach the completed form to their tax return to claim the child for tax benefits, including the child tax credit.

Required documents needed to accurately prepare your tax return:

  • Unexpired Government-issued Photo ID (Taxpayer & Spouse)

  • Birth Certificates (for all dependents)

  • Social Security Cards (for all dependents)

  • Prior year tax return

  • All tax forms (W-2, 1099, etc.)

  • Proof of expenses

  • Supporting documents

Please upload a valid photo ID for yourself and spouse (if filing jointly). Other required documents can be uploaded at a later time through our designated secure portal.

I understand that incomplete or inaccurate information may delay filing or result in IRS correspondence such as changes to your tax liability, penalties, or disallowance of certain credits. 

I understand that MG Coaching must follow IRS due diligence requirements by asking the appropriate questions to verify your eligibility for any tax credits claimed, and may request additional documents if needed.
