I acknowledge that Longevity DSM has explained the nature and purpose of the treatment and the potential risks/complications/side effects. I understand that results vary by individual and that no guarantees have been made regarding effectiveness, duration, or outcomes.

I confirm there have been no changes to my medications, supplements, or health conditions since my last visit, and I agree it is my responsibility to abide by my plan of care. I will inject my medication at the dose, frequency, and set day as directed by my plan of care and medical team. I will contact the medical team prior to changing my dose amount or injection day. I have had the opportunity to ask questions, received prior instruction on self-administering injections, and do not need additional education. I also understand that medications must be refrigerated and, once dispensed, cannot be returned due to safety requirements.
