By signing and consenting to this Private Ecclesiastic Contractual Agreement, Covenant, Treaty, Informed Consent, Membership Agreement, and Oath, I hereby enroll as a member of the Nuri Trust Consortium—an international spiritual-scientific care and counseling research trust™ approved by the Global Commission for Accreditation of Spiritual Medicine (GCASM) and as a member of a Private Membership Association (PMA).

I understand that I am agreeing to enter into a PMA agreement as member of Restorative Health Ministries PMA and entering a private trust joint-venture agreement with the master trust consortium under which this entity operates. I understand that as a member, any and all disputes, claims, or grievances involving the Nuri Trust Consortium—its authorized representatives, constituent parts, and joint ventures—must be brought exclusively before the Private Ecclesiastic Arbitration Committee and Board of Trustees of the Nuri Trust Consortium for resolution.

I agree that the Nuri Trust Consortium retains full and exclusive privilege of private ecclesiastic jurisdiction over such disputes, and that no external or civil jurisdiction shall supersede this authority. I acknowledge that the Arbitration Committee and Board of Trustees will make every good-faith effort to resolve any and all matters brought before them. I further understand and agree that the format, methodology, structure, and content of this agreement are proprietary, constitute protected intellectual property of the Nuri Trust Consortium, and may not be used, duplicated, altered, disseminated, or transmitted in any form without express written authorization. This document becomes valid and legally binding only upon approval by an authorized representative of the research trust committee.
