Permanent Jewelry Service Waiver and Consent

Thank you for choosing LaBrows Beauty Studio, LLC for your permanent jewelry. Permanent jewelry involves welding a piece of jewelry (such as a bracelet, anklet or necklace) directly onto you without a clasp, creating a continuous piece. This waiver explains the service and asks for your consent.

Permanent Jewelry: Consent for Service *

I am voluntarily receiving the permanent jewelry service provided by LaBrows Beauty Studio, LLC. I understand this service uses welding equipment to permanently secure a piece of jewelry on my body.

Permanent Jewelry: Welding Process *

I acknowledge the process involves minimal heat and that there may be minor risks, including (but not limited to) brief discomfort, minor burns, skin irritation or other unforeseen reactions.

Permanent Jewelry: Release of Liability *

I understand and agree that LaBrows Beauty Studio, LLC and its employees, agents and representatives are not responsible for any injury, harm or allergic reaction that may occur as a result of this procedure or the materials used. I agree to release and hold harmless LaBrows Beauty Studio, LLC from any claims or liabilities related to this service.

Permanent Jewelry: Material Information *

The materials used are solid gold, gold-filled and its base metals, and 925 sterling silver. I have been informed of the metals used, and it is my responsibility to tell my technician about any known allergies or sensitivities.

Permanent Jewelry: Aftercare and Removal *

I understand permanent jewelry is intended to be long-lasting and removing it may require cutting it. LaBrows Beauty Studio, LLC is not responsible for removal or any associated costs, including damage to the jewelry if removal is necessary.

Permanent Jewelry: Risks and Responsibilities *

It is my responsibility to care for the jewelry properly and to be cautious during activities that could harm it (e.g., sports, heavy lifting or exposure to harsh chemicals).

Permanent Jewelry: Cancellation and Refunds *

Once the service is completed, there are no refunds or exchanges. If I need to reschedule or cancel, I agree to follow the LaBrows Beauty Studio, LLC cancellation policy.

Permanent Jewelry: Medical Conditions *

I confirm I do not have any medical condition (such as a metal allergy or skin condition) that would be negatively affected by wearing permanent jewelry, and I will tell my technician about any concerns before the service begins.

I voluntarily consent to receive permanent jewelry and assume all risks involved. I agree that my electronic signature is legally binding and has the same effect as a handwritten signature under the federal Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. 7001 et seq.) and the Texas Uniform Electronic Transactions Act (Texas Business and Commerce Code, Chapter 322).
