MEDICAL AUTHORIZATION AND LIABILITY RELEASE
I hold VIM GYM LLC owners, coaches, and staff harmless for any injuries arising out of participation in any and all classes or activities from or at the gym.
I, the undersigned parent or legal guardian, authorize VIM GYM and its owners, coaches, employees, and designated representatives to obtain emergency medical assistance for my child if I cannot be reached or if immediate action is reasonably necessary.
In the event of an injury, illness, or medical emergency, I authorize VIM GYM representatives to contact emergency medical services (911), request an ambulance, and facilitate transportation to an appropriate medical facility when deemed necessary. I understand that VIM GYM staff are not medical professionals and will not diagnose or provide medical treatment beyond their training.
I understand that I am responsible for all costs associated with medical evaluation, treatment, ambulance transportation, and related services.
I agree to provide accurate and current emergency contact information, disclose relevant medical conditions, allergies, injuries, activity restrictions, and other information necessary for my child’s safety, and notify VIM GYM of any changes.
ASSUMPTION OF RISK
I and the participant(s) understand the nature of this gym's activities and sponsored events and certify that the participant(s) is qualified, in good health, and in proper physical condition to participate in such activity. I understand that the sport of cheerleading involves risks or serious bodily injury in the activities performed at this facility, as well as in travel. I and participant(s) fully understand that this activity involves those actions of others participating in the event, the conditions in which the event takes place, or the negligence of the "releases" named above; and there may be other risks either not known to me and or participants or not readily foreseeable at this time. In recognition of this acknowledged risk of injury, I knowingly and voluntarily waive all rights and/or causes of any kind, including any and all claims of negligence arising as a result of such activity from which liability could accrue to VIM GYM,LLC.
RELEASE
As the parent, self, or legal guardian of the child(ren) listed above, I hereby give my full consent and approval for my child(ren) to participate in the classes and programs at VIM Gym, LLC. I understand the coaches and staff are not held accountable for injury or serious bodily injury.
PHOTOS/VIDEOS
I hereby give VIM GYM, LLC permission to display photos or videos of my child(ren), without names, on website, social media and promotional materials.
