To begin booking a lesson, please fill out the form below. We will contact you within 48 business hours to schedule your booking. All lessons require payment before confirming the booking.
Assumption of Risk:
I have chosen for myself and/or my child to participate in classes, private lessons, rehearsals, workshops, performances, competitions, events, and other activities provided by Ovation Dance (“Ovation”). I understand that participation in dance, acrobatics, tumbling, conditioning, and related physical activities involves inherent risks, including but not limited to falls, collisions, strains, sprains, illness, and other injuries.
I voluntarily assume all risks associated with participation and hereby release and hold harmless Ovation, its owners, directors, instructors, staff, contractors, and volunteers from any liability, claims, demands, damages, injuries, illnesses, losses, or expenses arising from participation in Ovation activities or use of its facilities.
Medical Authorization:
I authorize Ovation staff to obtain emergency medical care for the participant if necessary. Every reasonable effort will be made to contact the participant and/or parent/guardian emergency contact prior to treatment. I grant permission for qualified medical personnel to provide care deemed necessary during participation in Ovation activities.
Supervision Policy:
I understand that Ovation does not provide childcare or supervision outside of scheduled class, rehearsal, lesson, or approved program times.
I understand that some private lessons or specialty instruction may be scheduled directly with instructors; however, all participants remain subject to Ovation policies and waiver terms while using Ovation facilities or participating in Ovation activities.
Media Consent:
I grant permission for photos and/or videos taken during Ovation activities to be used for promotional purposes, including social media, website content, advertising, print materials, and other studio-related marketing without compensation. If I choose otherwise, I must submit my exclusion form in writing
Studio Policies:
I/we agree to follow all studio policies and understand that Ovation reserves the right to remove or suspend any participant whose behavior is disruptive, unsafe, inappropriate, or unable to follow studio expectations. No refunds will be issued in these situations.
