Cancel Request Form

Your contract requires a written notice to cancel. You are responsible for all fees until your membership is canceled. You acknowledge that all cancellations take place on the 28th of the following month. The draft date initiated by the gym for your last month's dues may differ from your financial institution's posting date. You will have access to all the benefits of the gym until the completion of your membership

I understand that my account will be canceled the following month on the 28th, unless I elected to pay my last month's dues upfront at the start of my membership. Only memberships established January 2024 or later had this option.

For example, it your bill typically cycles on the 15th of the month and you cancer on the 14th of January, you will be billed on 15 January and 15 February as that is your following month. You will have access to the facility and the services aligned with your membership until 14th of March.
