Agreement confimation: *

I understand that submitting this form is an offer, not a guarantee of acceptance. The Patieaux Chick will review my donation and may accept, decline, or accept part of it. Once accepted, my donation belongs to The Patieaux Chick. It may be used at Holidays at LaTerré Patieaux on Saturday, December 12, 2026, at a future community event, or given to families in need, at The Patieaux Chick’s discretion. I confirm I have the right to donate what I’m offering, and that donated items are new or in excellent condition, safe, and not recalled. Food donors are responsible for safe preparation, handling, and any required permits. I’m responsible for delivery unless the team agrees otherwise in writing. The estimated value I entered is my own. The Patieaux Chick does not appraise or confirm the value of any donation, and my donation is not tax-deductible unless The Patieaux Chick confirms otherwise in writing. Anyone providing donated services does so as an independent provider, not an employee, and is responsible for their own staff, equipment, and insurance. Any monetary gift I add is non-refundable. I grant The Patieaux Chick permission to use my name, business name, and logo on event materials, the website, social media, and future promotion, at a recognition level The Patieaux Chick decides. I confirm I have the rights to any logo I upload. Checking this box is my electronic signature. This agreement is governed by Texas law.
