Liability Waiver: By checking this box, I am aware that there may be an inherent risk of injury while participating in dance and fitness classes. Though every safety measure is being practiced, I understand the possibility of these risks and agree to indemnify and hold harmless 4.0. Movement Studios (4.0). 

I am aware that I am providing my information and photos from class at my own free will. I am also aware that photos, voice, video images, and/or recordings will be taken during classes for marketing purposes. I am aware that photos and videos taken at the event will become the sole property of 4.0 and posted on several websites/social media sites of 4.0. 4.0. is not responsible for photos, information, or video images that are taken from our websites/social media page(s) and posted on other public viewings. I permit my photos and video images to be posted on the above entities' website(s) and social media page(s).

I also understand that there are no refunds on classes purchased, but credits are given for other classes on a case-by-case basis.

Refunds: By checking this box, I am aware that 4.0. Movement Studios does not offer refunds on tuition, registration fees, classes, workshops, costumes, performance tickets, merchandise, or any other services. In the event of a withdrawal, students may complete the number of classes already paid for within the current season or attend make-up classes, when available. Credits may be issued at the discretion of our administrative staff in cases of emergencies; however, credits are non-refundable, non-transferable, and must be used within one year of the issue date. Please note that no refunds will be given on account credits, nor can payments be transferred to another student or family member for unused classes
