Stretch Therapy Consent Form
BY SIGNING BELOW, YOU ACKNOWLEDGE AND AGREE TO THE FOLLOWING:
Stretch therapy is not a substitute for medical examination or diagnosis. It is recommended that I see a physician for any physical ailment that I may have. I understand that the Stretch Therapist does not prescribe medical treatments or pharmaceuticals and does not perform any spinal adjustments. I am aware that if I have any serious medical diagnosis, I must provide a physician’s written consent prior to services.

It is your responsibility to inform the Stretch Therapist of any discomfort you may feel during the session so they may adjust accordingly. You will not hold the Stretch Therapist responsible for any pain or discomfort you experience during or after the session.

The client consents to arriving in appropriate clothing for session (tight workout garment). The Stretch therapist does not engage in breast massage of female clients unless the client gives written consent before each session involving breast massage.

The Stretch therapist must immediately end the session if a client initiates any verbal or physical contact that is sexual in nature.

If the client is uncomfortable for any reason, the client may ask the Stretch Therapist to end the session, and the Stretch Therapist will end the session. The Stretch therapist also has a right to end the session if uncomfortable for any reason.

If you're late, it is the Stretch Therapist's discretion whether you receive your full time.

No-shows or cancellations within 24 hours of the scheduled appointment get charged the entire original service amount. And this includes rescheduling or wanting to shorten your session. Being over 30 minutes late or inappropriate is an automatically charged cancellation. There are no refunds if a client decides to cancel a session while its ongoing.

By signing this release, you herby waiver and release Minion Training LLC from any liability, past, present, and future relating to stretch therapy and bodywork.
