By signing this Release of Liability, you, on behalf of your student, hereby acknowledge and assume all risk and danger incidental to dance class, community days including, but not limited to: slips, trips, falls, collisions with other participants, and physical strain or injury. You hereby, for yourself, your heirs, executors, assigns, and administrators, waive, release, and forever discharge Mountain Movers Creative Arts, LLC (“MMCA”), their respective agents, sponsors, representatives, successors, and assigns from any and all claims, demands, actions, or rights of action either in law or in equity, arising from or by reason of any bodily injury, death, or property damage resulting from you or your children(s)’ participation in exercise programs or activities of any kind in connection with MMCA.



This waiver applies to this visit and all future visits.
