This PACK POLICY FORM shall be legally binding between the CLIENT and the TRAINER. By signing below and attending or participating in any training sessions, the CLIENT is agreeing to the conditions laid out in the above PACK POLICIES and has had a chance to express any and all concerns either verbally or in written form with the TRAINER. The above agreement releases Wolfman's Canines LLC, its staff, owners and any representatives from any and all liability which the CLIENT or their dog(s) may suffer including but not limited to injury, sickness, damage, or death resulting from participation in training or boarding. No waiver or modification of any provision herein shall be valid unless expressly agreed to in writing by both the CLIENT and Wolfman’s Canines LLC. *
