
Terms and Consent
I hereby consent to receive physical therapy services from Pin-Point Physical Therapy, LLC and its licensed providers. These services may include, but are not limited to:
Orthopedic and sports physical therapy
Manual therapy (soft tissue mobilization, joint mobilization, instrument-assisted techniques)
Dry needling (if clinically indicated and verbally discussed)
Therapeutic exercise and mobility training
Neuromuscular re-education
Taping, bracing, or corrective exercise instruction
Use of exercise equipment, resistance bands, steps, weights, sliders, or other tools
I understand that physical therapy carries some inherent risks, including soreness, muscle fatigue, bruising, dizziness, or temporary symptom aggravation. I agree to communicate any discomfort, concerns, or changes in my condition to the therapist immediately.
I understand that treatments may be provided outside of a clinic setting, including but not limited to:
Running races or trail running events
Skiing events or preseason screenings
Gym classes or group workout sessions
Corporate wellness events
Community fitness/health events
Kayaking, outdoor recreation, or sports-specific workshops
I acknowledge that these environments may involve variable surfaces, weather conditions, equipment limitations, or surrounding participants. I agree to follow all safety instructions and understand that participation is voluntary.
I understand that participation in physical therapy, movement screenings, athletic testing, or exercise-based treatment may involve physical exertion. I voluntarily assume all risks associated with participation and agree that I am medically able to take part in these activities.
I understand that it is my responsibility to inform the therapist of:
Any pre-existing medical conditions
Any recent surgeries or injuries
Any changes in symptoms
Any recommendations from a physician
In consideration of receiving physical therapy services from Pin-Point Physical Therapy, LLC, I agree to release, indemnify, and hold harmless:

Nick Anderson, PT, DPT
Any employees, contractors, event volunteers, or partners
from any liability for injury, illness, or damages that may result from participation in treatment, screenings, or movement-based activities—except in cases of gross negligence or misconduct.
