I/We declare that all information given in this application is correct and hereby authorize Parkview to verify any or all of the information contained herein. Upon submission, the application and any supporting documents become the property of Parkview and will be protected in accordance with the Personal Information Protection and Electronic Documents Act (PIPEDA). I/We hereby consent to the collection, use, and disclosure of this personal information by Parkview for the purposes of evaluating my/our application and offering me/us residential accommodation.

I/We understand that this application does not constitute an agreement on the part of Parkview to provide me/us with housing, nor does placement on the waiting list guarantee accommodation. Parkview reserves the right to amend or change its intake process, including but not limited to the imposition of a fee for placement on or maintenance of the waiting list, at its sole discretion. Applicants will be notified of any such changes.

I/We understand that Parkview may revise eligibility requirements for tenancy at its sole discretion. Any such changes will be communicated to those on the waiting list, and continued eligibility will be assessed based on the new requirements.

I/We understand that if Parkview is unable to contact me/us using the provided contact information on three separate occasions, I/we will be automatically removed from the waiting list. It is my/our responsibility to ensure that the contact information provided remains current and accurate.

I/We agree to contact Parkview annually to confirm continued interest in accommodation. Failure to do so within 30 days of the annual notification request may result in the removal of my/our name from the waiting list without further notice.

I/We understand that Parkview will primarily use the contact details provided (email, phone, or mailing address) for communication. It is my/our responsibility to ensure that Parkview is informed of any changes to my/our contact information. Parkview will not be responsible for missed communications due to outdated contact details.

I/We agree that if housing is provided, it will be occupied only by the persons listed on this application who meet all applicable eligibility requirements. Applicant substitutions are strictly prohibited without the prior written consent of Parkview and will void this application.

I/We understand that should I/We decline three offers of housing, my/our name will be placed at the bottom of the current waiting list.

I/We understand that Parkview’s Life Lease Housing and Parkview Village Apartment Rentals is independent living, and any medical or care support services are provided through third-party partnerships and are the responsibility of the Resident to arrange.

I/We understand that Parkview has a no-smoking/vaping policy that will be abided by as covered in the rules and procedures.

I/We acknowledge that Parkview is relying on the truth of the statements and representations I/We have made in this application. Should any of the information provided be untrue, I/We understand that my/our application may be voided at the sole option of Parkview. 

 I/We acknowledge that Parkview reserves the right to refuse housing to any applicant who no longer meets the eligibility criteria at the time a unit becomes available, or who has failed to maintain up-to-date information, including contact details
