Consent and Agreements

I/We hereby grant permission to the rights of my/our child's image, likeness, and sound of their voice as recorded on audio or video tape without payment or any other consideration. I/We understand that my/our child's image may be edited, copied, exhibited, published or distributed, and waive the right to inspect or approve the finished product wherein my/our child's likeness appears.

I/We, the undersigned, acknowledge that participation in sports entails known and unanticipated risks that could result in physical or emotional injury, paralysis, death, or damage to my/our child, to property, or to third parties. I/We expressly accept and assume all of the risks inherent in this activity or that might have been caused by the negligence of the program staff.

I/We consent to the collection, use, and storage of my/our personal information as provided on this form for the purposes of participation in the basketball program. I/We understand that the information will be used exclusively for the purposes related to the event and will be kept secure and confidential in accordance with our CRM's data privacy standards.
