I understand that termination of a discounted membership before the end of the membership term will require an early cancellation fee of the difference between the membership I was on and the membership that reflects the time I was an active member.

I understand all memberships need 30 days' notice, starting from the date I submit this form.

I understand any payment due during my notice period is taken in full, and I can keep training until it ends.

If my membership is on hold, I understand it must come off hold before it can be cancelled.
