1. 註冊易帳戶授權 Authorization for e-Registry Account
各董事/股東同意授權 CatcherBiz Limited 用閣下身份登記「註冊易」賬戶，有關帳戶只用於辦理開立公司用途。
The directors/shareholders hereby authorize CatcherBiz Limited to register an "e-Registry" account using their personal identity. Such account shall be used solely for the purpose of company incorporation.

2. 資料真實性及披露 Declaration of Information and Disclosure
本人謹此聲明在本表格中提供的資料是完整及真實，並保證在擔任公司董事/股東/重要控制人期間，如有任何資料變更或出現影響資料準確性或完整性的情況，本人會於變更後 30 天內向 CatcherBiz Limited 作出通知。本人在此同意 CatcherBiz Limited 可根據相關的法律要求使用及/或向第三方披露本人之資料。

I hereby declare that the information provided in this form is complete and correct and I undertake that, as long as I continue to be a director/shareholder/significant controller of the Company, I will notify CatcherBiz Limited within 30 days of any changes to or affecting the accuracy or completeness of the information provided. I hereby CONSENT that, subject to relevant legal requirements, CatcherBiz Limited may use and/or disclose my information to third party(ies).
