CONTRACT FOR VEHICLE REPAIR AND REPAIR AUTHORIZATION
1. Authorization
I authorize Body Works Plus (“Repair Facility”) to inspect, disassemble, diagnose, and repair my vehicle. I confirm that I have the legal right to authorize these repairs. I also authorize the Repair Facility to photograph, scan, test drive, move, and otherwise operate the vehicle as necessary to complete repairs and verify safety and functionality. I understand that all repair documentation, including estimates and repair plans, remain the property of Body Works Plus.
2. Payment Responsibility
I agree to pay all charges related to the repair of my vehicle, including parts, labor, materials, taxes, fees, diagnostics, administrative costs, and any applicable towing, storage, or parking fees. Full payment is required prior to vehicle release unless otherwise agreed in writing.
I understand that any insurance estimate is not a final bill and does not determine the proper or complete repair of my vehicle.
> I UNDERSTAND THAT I AM PERSONALLY RESPONSIBLE FOR ALL REPAIR COSTS, EVEN IF MY INSURANCE COMPANY DOES NOT PAY ALL OR PART OF THE CLAIM.
3. Repair Process & Additional Damage
I understand that additional damage may be discovered during the repair process. If so, Body Works Plus will notify me of any additional repairs and associated costs. I authorize supplemental repairs necessary to restore my vehicle to proper and safe condition.
4. Insurance Acknowledgment
I understand that Body Works Plus is not obligated to follow an insurance company’s estimate or repair recommendations. The Repair Facility will determine the proper and safe repair methods. Any communication with my insurance company is provided as a courtesy and does not make Body Works Plus my agent.
> I understand that my insurer may not cover all necessary repairs and that I am responsible for any remaining balance.
5. Vehicle Possession & Lien
I understand that Body Works Plus has the right to retain possession of my vehicle until all charges are paid in full and may exercise lien rights as permitted by law.
>I agree not to transfer ownership or title of the vehicle until all repair costs have been paid in full.
6. Parts Authorization
Body Works Plus may use OEM (Original Equipment Manufacturer) parts when appropriate. If I elect or approve the use of alternative parts, I understand this may affect vehicle value, warranties, or lease agreements.
7. Workmanship Warranty
Body Works Plus guarantees its workmanship for as long as I own the vehicle. This warranty applies only to labor performed and does not include parts or materials. I must notify the Repair Facility and allow an opportunity to correct any issues.
8. Storage & Abandoned Vehicle
I agree to pick up my vehicle promptly after being notified that repairs are complete. Vehicles not picked up within a reasonable time may incur storage fees.
If the vehicle is abandoned or unpaid, I understand additional fees, legal action, or disposal procedures may apply as permitted by law.
9. Limited Authorization for Insurance Payments
I authorize Body Works Plus to receive and process insurance payments related to my vehicle repairs. This does not relieve me of my responsibility to pay any remaining balance.
10. Entire Agreement
This agreement represents the full understanding between the parties and may only be modified in writing.
BY SIGNING THIS DOCUMENT I GIVE AUTHORZATION AND POWER OF ATTORNEY TO BODY WORKS PLUS AS STATED ABOVE.
