I have read and agree to the terms of CrossFit Cygnus' Liability Waiver. I agree to purchase my membership at the agreed upon rate and contract term. I understand my account will be billed on the same day each month for the extent of the membership agreement. I understand that membership dues are non­refundable and paid in advance of service each month. I also understand that at the end of my membership agreement, my contract will automatically renew on a month-by-month basis at the current rate unless I provide written notice at least five (5) days prior to my billing date of my intention to cancel.
