Mandatory Pickup/ Drop-Off Policies and Procedures

• All students must be dropped in and picked up from within the facility and must be signed in and out by the authorized parent/guardian.

  
  

• GEM Academy will not release the student to anyone, not on the authorized pickup list.

  
  

• In case of an emergency, an email containing the emergency pickup person's Full name, Email, Phone number, and Relation to the child must be sent to [email protected] from the on-file authorized person's email address. (ID must be shown at the front desk for verification)

• Liability Waiver: I hereby state that my child is in good mental and physical health condition to participate in all GEM activities and classes. I hereby release GEM Multicultural Center from any and all liability, claims, suits, demands, judgments, costs, interest, and expenses (including attorney fees and costs) arising from, or related to, such activities and/or any related activities, including any accident or injury to the student and the costs of medical services. I also agree not to hold these parties responsible in the event that myself/son/daughter engages in inappropriate conduct (including, but not limited to disruptive or volatile behavior in or out of the center, etc.) or become involved in any activity or with any persons not associated with this program. I further attest that the information in this application is correct to the best of my knowledge. I hereby allow myself/child to be photographed and videotaped at GEM Center for the purposes of the Center’s promotional material and publications. I hereby grant consent for the use of my child's academic data for educational and research purposes. This data may include, but is not limited to, assessment results, progress reports, and any other academic information relevant to my child's educational journey.
