Participation & Liability Waiver

Assumption of Risk

I understand that participation in dance classes, rehearsals, performances, camps, and other related events at Music in Motion Dance Studio (“the Studio”) involves physical movement and carries the risk of injury or illness, including but not limited to sprains, strains, falls, and exposure to communicable diseases such as MRSA, influenza, and COVID-19. I acknowledge that while studio rules, equipment, and staff supervision help minimize these risks, they cannot eliminate them entirely.

Release of Liability

In consideration of myself or my child being allowed to participate in any Music in Motion programs or activities, I, for myself, my child, my spouse, and our heirs, executors, and assigns, hereby release and hold harmless Music in Motion Dance Studio, its owners, directors, officers, employees, instructors, agents, volunteers, and landlords (collectively “the Releasees”) from any and all liability for injury, illness, disability, death, or property damage that may result from participation, whether arising from the negligence of the Releasees or otherwise, to the fullest extent permitted by law.

Indemnification

I agree to indemnify and hold harmless the Releasees from any and all claims, demands, damages, costs, or expenses (including attorney’s fees) arising out of or related to my or my child’s participation in Studio activities.

Medical Treatment

In the event of injury or illness, I authorize Music in Motion Dance Studio staff or representatives to obtain medical care deemed necessary for myself or my child and accept full financial responsibility for such care.

Acknowledgment of Rules

I agree to abide by all studio policies and safety procedures and to notify the Studio immediately of any health conditions or concerns that may affect participation.

Consent & Understanding

I have read this waiver in full, understand its contents, and sign it voluntarily. I acknowledge that I am giving up certain legal rights, including the right to sue for injuries or damages resulting from ordinary negligence.
