Trust Information

 The following information is used to create an NFA Gun Trust for your NFA firearms through the 199Trust system. Please ensure all fields are completed and everything is current and accurate.

Utah FAST is not responsible for errors on the NFA Trust if this form is not completed correctly.

Note: If you have already submitted a copy of your trust docs along with a Form 1 or 4 to the ATF, you will need to use the 'common amendment examples' document sent with your trust email to amend your original trust. The change request fee is $10 after the 10-day inspection period.

199Trust Change Form

E-mail delivery of NFA Trust documents within 48 hours.

This is the name of the person initiating the Trust, and is likely the person filling out this form. First, Middle, Last, Suffix (if applicable)
e.g. Salt Lake, Tooele, Summit, etc.
This is the state that which the Trust will be initially formed, .e.g. Utah.
CO-TRUSTEES MUST BE 18 YEARS OR OLDER, AND BE ABLE TO LEGALLY POSSESS FIREARMS. A Co-Trustee is someone that you would like to allow to handle/use/transport items placed in the Trust. By adding Co-Trustees you would like to allow them to have access to the items/store the items, and have the ability to add items.

*** A Co-Trustee can also be a Successor Trustee and a Beneficiary ***

  • ALL Co-Trustees must sign the Trust when it is notarized.

  • ALL Co-Trustees must attach fingerprints, a passport photo, and an ATF Form 23 (Responsible Person Questionnaire) each time a new Form 1 or Form 4 is submitted to the ATF.

You can add them later, after the NFA firearm is approved and transferred into the Trust.

It is necessary to have a Successor Trustee in case the current Trustee(s) for whatever future reason does not meet the requirements to possess NFA controlled items/firearms. If Co-Trustees are included, the Successor Trustee would only act if the other Trustees all became ineligible and the Settlor did not elect another Successor Trustee. **A Co-Trustee (other than the Settlor) can also be named as a Successor Trustee** *Neither successor trustee nor beneficiary requires signatures. Beneficiaries are people (or organizations, etc.) that you would like the items in the Trust to be distributed to upon the death of the Settlor. DO NOT list yourself (settlor) as a beneficiary (or successor trustee). Please add one name per line. First, Middle, Last, Suffix (if applicable)
Typically this is the last name of the Settlor and then the word "Trust". For example, if the Settlor's last name is Anderson, the Trust would be named "Anderson Trust". However, you can name it anything you'd like. Just remember that shorter might be better since you'll have to engrave it on anything you build on an ATF Form 1.
