Liability Waiver: I hereby state that my child is in good mental and physical health to participate in all GEM activities and classes. I hereby fully and forever release and discharge the Gem Advisers, d/b/a Gem Academy and Gem Multicultural Center, LLC, from, and expressly waive, any and all liability, claims, and demands of whatever kind or nature, either in law or in equity, that may arise from my participation in the activities. I agree not to make or bring any such claim or demand against the organization as listed above, and fully and forever release and discharge the organization from liability under such claims or demands. i understand that this release discharges the Gem Advisers, d/b/a Gem Academy and Gem Multicultural Center, LLC, from any liability or claim that I may have against the organization with respect to any bodily injury, personal injury, illness, death, property damage, or property loss that may result from the activities, whether caused by the negligence of the organization or otherwise. I also agree not to hold these parties responsible in the event that myself/son/daughter engages in inappropriate conduct (including, but not limited to, disruptive or volatile behavior in or out of the center, etc.) or becomes involved in any activity or with any persons not associated with this program. I further attest that the information in this application is correct to the best of my knowledge. I hereby allow myself/child to be photographed and videotaped at GEM Center for the purposes of the Center’s promotional material and publications.
GEM Academy reserves the right to modify the schedule or cancel the event in case of unforeseen circumstances.
Consent for Communication: I consent to receive communications via email or text message regarding this event and future events hosted by GEM Academy.
Parent/Guardian Permission (for minors): If under 18, I confirm that I have obtained parental/guardian consent to participate in the DSAT practice test.
Cancellation Policy: Due to limited seats, please notify us at least 48 hours in advance if you cannot attend the test so that others can participate.
Refund Policy: All fees paid are non-refundable under any circumstances. If you are unable to attend a session, you may request to be moved to another available session.
Reminder of Testing Materials: Please bring pencils, erasers, and a calculator to the test. Calculators will not be provided.
Follow-Up and Results: The DSAT practice test results will be emailed within two weeks of the test date. You may contact us at [email protected] with any questions.
