LIABILITY WAIVER, RELEASE OF CLAIMS, MEDICAL AUTHORIZATION, MEDIA CONSENT, AND CODE OF CONDUCT AGREEMENT

By signing below, I acknowledge and agree to the following terms and conditions on behalf of myself and/or my minor participant (“Participant”) in connection with participation in any Star Wrld 3x3 programs, events, leagues, camps, clinics, tournaments, trainings, practices, strength and conditioning sessions, travel activities, media activities, and related events.

This agreement applies to all Star Wrld 3x3 activities, including but not limited to:

• STAR WRLD 3x3 Leagues

• STAR WRLD Development Programs

• STAR WRLD Camps and Clinics

• STAR WRLD Tournaments

• STAR WRLD Media Activities

• STAR WRLD Strength & Performance Activities

• STAR WRLD Community Events

1. ASSUMPTION OF RISK

I understand that participation in basketball and athletic activities involves inherent risks, including but not limited to:

• Serious bodily injury

• Concussions

• Sprains and fractures

• Heat exhaustion

• Illness

• Paralysis

• Permanent disability

• Death

• Property damage

• Emotional distress
  

I voluntarily assume all risks associated with participation in Star Wrld 3x3 activities, whether known or unknown, foreseeable or unforeseeable.

I understand that these risks may result from:

• Athletic participation

• Physical contact with other participants

• Equipment use

• Facility conditions

• Transportation

• Weather conditions

• Negligence of other participants

• Negligence of coaches, volunteers, referees, staff, or spectators

I acknowledge that participation is completely voluntary.

2. RELEASE OF LIABILITY

In consideration for being permitted to participate in Star Wrld 3x3 activities, I hereby release, waive, discharge, and covenant not to sue:

• Star Wrld 3x3

• Coaches

• Volunteers

• Officials

• Referees

• Trainers

• Directors

• Event operators

• Sponsors

• School districts

• Facility owners

• Facility operators

• Independent contractors

• Partner organizations

• Employees

• Agents

• Representatives

• Successors and assigns

from any and all liability, claims, demands, damages, losses, causes of action, costs, or expenses arising out of or related to participation in Star Wrld 3x3 youth programs.

This includes claims arising from negligence to the fullest extent permitted by law.

3. MEDICAL AUTHORIZATION

I certify that the Participant is physically able to participate in athletic activities.

I authorize Star Wrld 3x3 staff, representatives, or emergency personnel to obtain medical treatment for the Participant in the event of illness, injury, or emergency.

I understand and agree that:

• Emergency medical treatment may be provided if deemed necessary.

• I am financially responsible for all medical expenses incurred.

I understand that it is my responsibility to disclose any medical conditions, allergies, medications, restrictions, or health concerns prior to participation.

4. MEDIA RELEASE

I grant Star Wrld 3x3  permission to photograph, record, livestream, and otherwise capture the Participant’s image, likeness, voice, appearance, performance, and name during participation in Star Wrld 3x3.

I understand and agree that Star Wrld 3x3 may use these materials for:

• Social media

• Marketing

• Advertising

• Promotional materials

• Websites

• Livestreams

• Broadcasts

• Commercial use

• Sponsorship materials

• Recruiting materials

• Documentary and storytelling content

I understand that no compensation will be provided for use of such media.

I waive any right to inspect or approve finished media products.



5. CODE OF CONDUCT

Participants, parents, guardians, coaches, and spectators are expected to conduct themselves in a respectful and sportsmanlike manner at all times.

The following behaviors may result in removal from activities without refund:

• Fighting

• Bullying

• Threatening behavior

• Unsportsmanlike conduct

• Disrespect toward staff, referees, coaches, players, or spectators

• Harassment

• Use of drugs, alcohol, or tobacco products

• Intentional damage to facilities or property

• Disruptive behavior
  

Star Wrld 3x3  reserves the right to suspend or remove any participant or spectator at its sole discretion.



6. TRANSPORTATION

I understand that Star Wrld 3x3 is not responsible for transportation to or from activities unless specifically stated otherwise in writing.

I acknowledge that any transportation arranged by parents, guardians, or third parties is done at their own risk.



7. REFUND POLICY

I understand that registration fees are generally non-refundable unless otherwise approved by Star Wrld 3x3 Directors in emergency situations.

Star Wrld 3x3 reserves the right to modify schedules, divisions, locations, or event structures.



8. FIBA 3x3 PLAY STYLE ACKNOWLEDGEMENT

I understand that Star Wrld 3x3 leagues and events may follow modified FIBA 3x3 rules and structures.

I acknowledge that:

• Gameplay may differ from traditional 5-on-5 basketball.

• No live coaching may be permitted during games.

• Officials and league operators have final authority regarding gameplay and event operations.



9. COVID-19 / COMMUNICABLE ILLNESS ACKNOWLEDGEMENT

I understand that participation in sports activities may expose the Participant to communicable illnesses, including but not limited to COVID-19.

I voluntarily assume all risks related to exposure or infection.

I agree not to participate if the Participant is experiencing symptoms of illness.



10. ACKNOWLEDGEMENT AND AGREEMENT

I certify that:

• I have carefully read and fully understand this agreement.

• I understand that I am giving up substantial legal rights.

• I voluntarily agree to all terms stated above.

• I am at least 18 years old OR the legal parent/guardian of the Participant.

• I understand this agreement is binding upon myself, the Participant, family
