This Add-On Agreement is incorporated into and forms part of the Skin Maintenance Membership Agreement. All terms, policies, payment authorizations, cancellation rules, and liability provisions of the Membership Agreement remain in full force and apply to the selected add-ons unless expressly stated otherwise.
