Werdum Training Center Waiver (Muay Thai)

I hereby certify that I have read and agree to each of the following provision and agree to be bound by the terms herein;

1 – I understand that all the activities offered at this facility, (BJJ, BJJ kids, NO GI, MMA, MUAI THAI, CAPOEIRA, KIDS CAPOEIRA & GINASTICA NATURAL,TRX, PERSONAL TRAINING, PRIVATE CLASSES, MMF), have martial art aspects, that also are system of unarmed fighting techniques.

2 – I understand that these activities will require me to engage in strenuous physical exercise, either individually or in class with more students and instructors, that the techniques learned are capable to cause minor and serious physical injuries including (abrasions, bruises, bleeding, broken bones, damage cartilages, tissues and organs), possibly emotion, psychological, risk of death and property damages, whenever used against another person or physical objects, either at the academy or elsewhere.

3 – I understand that my participation in class at the academy will require me to engage in conditioning exercises, formal exercises and other physical activities.

4 – I understand that the injuries described on paragraph #2may result not only from intentional action, but also inadvertent, negligent or reckless action by me or others including the malfunctioning or failure of any of the equipment including (pads, protectors, punching bags and tumbling mats) errors or mistakes on instructions  of performing fight, self defense techniques by instructors or students.

5 – I understand that the risks of any all types of injuries described in paragraphs 2-4 cannot be significantly reduced or eliminated without adversely affecting and even jeopardizing the nature and quality of the instruction of the activities at the academy.

6 – I understand that neither the Academy, owners, operators, officers, employees, instructors, students authorize me to use any of the fighting techniques and or defense techniques that I learn and study at the Academy against others, in any case or circumstances other than (A) - Martial arts training, matches, exhibitions or demonstrations in which my instructors and I agree that I am sufficiently  qualified and physically able to participate at the Academy or elsewhere (B) – Self-defense or defending others in a situation that involves the risk of imminent physical injury or death.

7 – I the light of the forgoing understandings, I knowingly and voluntary agree to assume all of the risks associated with the participation in classes at the Academy, including those described in paragraphs 2-4 and knowingly and voluntary hold harm-less and waive any and all rights to initiate or maintain a lawsuit or otherwise seek or obtain any finding of liability against the Academy, owners, operators, officers, employees, instructors, students or agents.

8 – I initiate this agreement, assumption of risks and waiver of risks, waiver of binding upon and any all of my parents, children, present and former spouses or life partners, heirs, assigns, friends, personal representatives and state in perpetuity.

(NOTE: ALL THIS INFORMATION IN THIS SECTION WILL BE TREATED STRICTLY CONFIDENTIAL AND IT WILL BE USED ONLY IN DETERMINATION OF THE APPLICANT FITNESS CONDITION TO PARTICIPATE IN MARTIAL ARTS ACTIVITIES WITH THE WERDUM TRAINING CENTER IN CASE OF EMERGENCY.)
