Guest Liability Waiver & Consent
I acknowledge that participation in this fitness class or activity carries the risk of injury. By signing below, I release Femnomenal Fitness, its staff, and affiliates from any liability for accidents, injuries, or damages sustained during participation.

If I am signing on behalf of a minor (under age 18), I certify that I am the minor’s parent or legal guardian and have the legal authority to provide this consent. I accept full responsibility for the minor’s participation and assume all risks associated with their involvement. I agree to release and hold harmless Femnomenal Fitness from any claims arising from the minor’s participation.

By signing, I accept full responsibility for my (or my child’s) participation and agree to follow all facility rules, including safety policies related to equipment and supervised areas.

I also consent to receive SMS alerts from Femnomenal Fitness regarding updates, events, or scheduling.
