What Is A Trauma Assessment?
A trauma assessment is a validated, trauma-informed tool used to confirm or rule out trafficking victimization. It also identifies trauma history, assesses current functioning, and provides individualized service recommendations.
Who Conducts These Assessments?
Establish has partnered with Ola Hou Clinic to coordinate referrals, while clinicians from Ola Hou Clinic, supervised by a licensed psychologist, conduct the assessments.
Who Can Submit A Request?
Requests can be submitted by service providers or by adults self-referring.
Is There A Cost?
No. These assessments are fully covered by a local community grant, so there is no cost to the individual or the referring provider.

Return the Referral Packet
Email the completed forms back to our team for review.
Assessment Setup
Our team sends the referral to Ola Hou Clinic. The assigned clinician will contact the individual or their parent/guardian if a minor to schedule the assessment.
