PLEASE READ:
1. If you're married to the other parent both usually have Parental Responsibility [PR]
2. If you're unmarried, the mother automatically has PR and the father has usually, provided the child was born after 01-12-2003 and jointly registered the birth with the mother
3. Those with PR can appoint Lifetime Guardians and if shared it should be done jointly
4. If your partner has PR for other children, they should complete an additional form because they will need a separate Lifetime Guardians Document
Use Of This Form:-
You can use this form for sole and shared PR for the same child or children
For any other minor children where you share PR with an ex-partner who is NOT the partner named here you will need two Lifetime Guardians Agreements - Please complete an additional form

PLEASE READ:
We strongly recommend appointing at least two sets of guardians—Primary and Secondary—as emergencies can happen anytime, however the better option is adding Tertiary Guardians because this gives even greater security to your children in case the other guardians cannot act
The same strategy applies to having three Trusted Persons - strength in numbers
We've just asked for a contact email for your Participants (Guardians, Trusted Persons, Witnesses) for your convenience we'll get the other required information directly from them when we ask for their permission to be appointed
