By signing this form you acknowledge that the fee is $250 for the community activity, and you are responsible for ensuring your NDIS plan covers the appropriate codes (04_104_0125_6_1 for In-House Activities and 04_210_0125_6_1, 09_008_0116_6_3 or 09_011_0125_6_3 for Community Activities). All Marching Forward team and representatives will always act in the best interest of your child. The natural environment carries inherent risk and whilst all attempts are taken to ensure safety you accept that we are not liable should your child sustain an injury during the sessions. Whilst attending an activity or STA/Respite participants are welcome to bring whatever makes them feel comfortable, we try to discourage expensive, tech items, but we also understand that some children need their items. As such, Marching Forward will endeavour to keep items safe, but we aren’t responsible for the damage or loss of any such items if the participant chooses to bring them. Marching Forward support workers are trained to handle many situations, and the groups safety is our number one priority so if we feel a situation is escalating and becoming unsafe for the group then we will ask the parent of the child to collect them from the location or a meeting point. Any cancellations within a week to 48 hours of an activity or STA/Respite will be charged at 50% of the rate, any cancellations 24 hours or less will be charged at the full rate, due to the nature of the activities and STA/Respite and the numbers for the third parties involved (accommodation, activities etc). If we can find another member to fill your place at short notice then we will endeavour to do so the cost can then be waived.
