I understand the nature of the activities offered by 360 Sports Centre LLC, including Cheer Kingdom and Parkour Kingdom, and give permission for the participant(s) listed above to participate. I acknowledge that activities involving movement, tumbling, stunting, parkour, inflatables, and other athletic activities carry an inherent risk of injury ranging from minor (such as bruises and sprains) to more serious injuries.

I agree not to hold 360 Sports Centre LLC, including Cheer Kingdom, Parkour Kingdom, its owners, employees, coaches, instructors, or volunteers, responsible for injuries or accidents that may occur as a result of participation. I understand this completed waiver must be on file before participating in any program or event.

If I have any questions or concerns regarding my child's safety or any aspect of a program, I will contact a member of the 360 Sports Centre leadership team immediately.

I also give permission for the participant(s) listed above to be photographed or videotaped for promotional and advertising purposes.
