Terms of Business

As a client of Faye Sheridan's HealthyHer Advanced Functional Health Coaching, I understand that:

  • This information is for educational purposes only and does not replace a physical diagnosis or link to any existing diagnosis.

  • The recommended supplements are not medicines.

  • Before starting any new exercise, supplement, or nutrition plan, I must consult my personal healthcare provider who has ultimate authority over my health and care.

  • Nothing discussed or recommended here is intended to influence or interfere with my healthcare provider's professional judgment or services.

Faye Sheridan shall not be liable for any:

  • Any loss

  • claim

  • liability, or

  • damage

That may be allegedly arising from information or recommendations made during consultations, or from materials provided within this document or correspondence.

I hereby release Faye Sheridan from any such claims.

It is common practice for certain practitioners to collect signatures on a liability waiver. By acknowledging this, I understand it's my responsibility to provide all laboratory test results, past and future, to my own physician for medical interpretation of any results provided by Faye Sheridan or her affiliates.

I agree to receive a nutritional interpretation of my test results from Faye Sheridan for my personal use as an educational tool for my health.

  • My personal physician can use these same results for diagnosis and treatment.

  • The information on Faye Sheridan's websites, brochures, flyers, and educational tools is believed to be accurate, but such accuracy cannot be guaranteed.

  • I release Faye Sheridan from any liability for injury or loss arising from the use of, or reliance on, the laboratory results and/or the dietary, supplement and lifestyle suggestions provided.

  • Before making any changes to my exercise, diet, or nutritional or hormonal supplementation, I will consult with my physician.

Faye Sheridan does not diagnose, cure, or treat any illness or disease. This information is not a substitute for a personal consultation with my own physician. I further release Faye Sheridan, her lab partners, independent representatives, associates, and affiliates from any liability for any failure to identify any medical condition or disease. We both understand this is not the purpose of their services.

Refund Policy

  • I am committing to 16 weeks of HealthyHer Advanced Functional Health Coaching.

  • I understand that there is no refund on the initial 16 weeks.

  • I understand that if I wish to cease continuing the HealthyHer Advanced Functional Health Coaching after the 16-week commitment, Faye Sheridan requires a two-week written notification that I no longer require her services

  • I understand that I will automatically be enrolled to the weekly Advanced Functional Health Coaching membership and will be charged AU$130 on a weekly basis should I fail to send my two-week notification as previously mentioned.

By submitting this form, I am acknowledging and agreeing to the Term of Business and Refund Policy as mentioned above.
