By submitting this order form, I confirm that all information provided is true, correct, and complete to the best of my knowledge.

1. Order Processing
All orders are subject to review and verification. Submission of this form does not guarantee approval. Orders will only be processed once all required documents have been reviewed and accepted.

2. Document Requirements

The borrower agrees to submit the following:
1. Borrower’s Valid ID (one (1) Primary ID, or Two (2) Secondary IDs if no Primary ID is available)
2. Co-maker’s Valid ID (one [1] valid ID)
3. Signed Contract
4. Proof of Income (If unavailable, a Barangay Clearance must be submitted)

Uploaded documents must be clear, readable, and valid. Blurred, expired, or incorrect documents may be considered incomplete.

3. Incomplete Requirements
If requirements are incomplete, missing, or invalid, we'll require a mandatory 20% down payment, calculated based on the total product price, before we can process your order.

4. Verification & Approval
All submitted information and documents are subject to verification. The company reserves the right to:
1. Request additional documents
2. Reject applications with false, misleading, or unverifiable information

5. Pricing & Availability
Product prices, availability, and terms are subject to change without prior notice. Final pricing will be confirmed upon approval.

6. Data Privacy
All personal information and uploaded documents will be treated as confidential and used solely for order processing and verification purposes, in accordance with applicable data privacy laws.
