Confirm that you have received, read, and understood each of the following obligations. These form part of your employment record.

Fair Work obligations
The Fair Work Information Statement (FWIS) and, where applicable, the Casual Employment Information Statement (CEIS) are legal requirements under the Fair Work Act 2009 (Cth).

I confirm that I have received and read the Fair Work Information Statement (FWIS) from Sharktech Global as required under the Fair Work Act 2009 (Cth).

If engaged as a casual employee, I confirm that I have received and read the Casual Employment Information Statement (CEIS), including information about my right to request conversion to permanent employment.

I acknowledge the award arrangement applicable to my role (as selected in Step 2) and understand the basis on which my remuneration has been set.

I acknowledge that I have been informed of my entitlements under the National Employment Standards (NES), including annual leave, personal and carer's leave, compassionate leave, parental leave, and notice of termination.

Work health and safety

I acknowledge my obligations under the Work Health and Safety Act 2011 (Cth) and applicable state legislation, including my duty to take reasonable care for my own health and safety and that of others.

I confirm that I will complete a WHS induction as directed by Sharktech Global before or on my first day, and will report any hazards, incidents, or near-misses to my manager immediately.
