1. Clinical Disclaimers & Limitations

By submitting this appointment request, you acknowledge and agree to the following:

  • No Active Correction: Retainers are passive appliances designed solely to hold teeth in their current position. They will not straighten crooked teeth, close gaps, or correct bite alignment issues.

  • Recent Dental Changes: If you have underwent recent dental treatment (e.g., new fillings, crowns, or implants) since your last scan or model was taken, the new retainer may not fit. Fabricating a retainer under these circumstances is done at the patient's own financial risk.

  • Dental Health Status: Ordering a replacement retainer online does not replace the need for regular routine dental check-ups. Newcastle Orthodontics assumes your dentition and gums are in a healthy, stable state suitable for retainer wear and by completing this form you affirm that you've had a checkup from a general dentist within the past 12 months verifying your mouth is healthy.

2. Patient Responsibilities

  • Fit Verification: If your new retainer feels excessively tight, painful, or does not fully seat over your teeth, you must cease wearing it immediately and contact the practice for an adjustment appointment.

  • Wear Protocol: To effectively maintain your orthodontic result, you agree to wear the retainer as previously prescribed by your orthodontist (e.g., every night). Failure to wear the retainer as directed will result in tooth movement (relapse).

3. Financial & Replacement Policy

  • Custom Fabrication: Because retainers are custom-made medical appliances tailored entirely to your unique anatomy, orders cannot be cancelled, refunded, or exchanged once laboratory fabrication has commenced.

  • Fit Guarantee Window: If there is a manufacturing defect or fit issue upon initial delivery, you must notify us within 7 days of receiving the appliance to arrange a clinical review or remake at no additional charge.
