We’re so excited that you’re taking the step toward baptism. Baptism at Union City Church is a joyful, communal celebration, and we love giving our church family the chance to hear and celebrate each person’s story.

After filling out this form, you’ll receive a link to write a short (4–6 sentence) testimony about why you’re choosing to be baptized. This testimony is typically shared on stage during the baptism service, but if you’d prefer, a member of our team can share it for you.
