Costume Fees *

I agree to the $100 recital costume fee. Each class has an assigned costume in the show. I understand that if my child is in multiple classes, they may require more than 1 costume and therefore more than 1 costume fee will be charged. Please note, some classes share the same costume (such as All Star pairings) and therefore not requiring more than 1 costume.

I understand that all costumes will be ordered for me by the studio and I am not responsible for placing any orders except for my child's required footwear for the stage.

I understand that costume fee(s) will be auto charged to the card on file on 9/15.

I understand costume payments received after 9/15 will be subject to an additional shipping charge.

I understand that I'll be notified when my dancer's costume is ready for pick up.

Participation Fee *

I agree to the $30 participation fee and understand that this fee will be auto charged on 9/15. I understand that recital participation is for actively enrolled dancers only and if my child cancels enrollment before show day, their recital participation will be denied.

Show Assignments *

Your child's class will either be assigned to Show 1 or Show 2. Both shows will be on the same day. Most teen classes perform in both shows. Although we do our best, we cannot guarantee that siblings will be in the same show as it is impossible to coordinate for all families. If your dancer is enrolled in multiple classes, we cannot guarantee that all of those classes will be in the same show.

Multiple Classes [Select One]

My child is enrolled in multiple classes and will be performing with all enrolled classes. I understand that (unless enrolled in an all star pairing) their participation may require multiple costumes as well as participation in both shows.

My child is only enrolled in one class. I understand they will only perform in one show and require one costume.

Parent Dance Participation [Select One] *

We will be participating in The Parent Dance. I understand that I will only perform in one show with my dancer (even if they're assigned to both shows). I understand my participation in this additional routine requires an extra $60 participation fee which covers rehearsal time. This fee will be auto charged to the card on file on 9/15. I understand the required rehearsal dates and agree to be in attendance.

We will not be participating in the parent dance.

Parent Dance is for ages 5+. Dancers younger than age 5 will not be eligible.

Refund/Withdrawal Policy *

I understand that there is a no refund policy for costumes, recital fees or show tickets even if there is a withdrawal of participation or dropped enrollment.
