• I authorize the camp team to seek medical assistance for my child in case of emergency.
• I authorize camp staff to make necessary medical decisions in the best interest of my child if I cannot be reached.

Waiver & Consent
• I confirm that my child is physically fit to participate in sports activities.

• I release the organizers from liability for injuries that may occur during the camp, except where caused by negligence.

Rules & Regulations
• Participants must follow coaches’ instructions at all times.
• Respect for staff and other participants is required.
• Proper sportswear must be worn.
• No bullying or unsafe behavior is allowed.
• Punctuality for drop-off and pick-up is required.
• No rescheduling or make-up sessions will be allowed once registration is confirmed.
• Missed sessions are non-refundable and cannot be rescheduled.
• Aggressive behavior, repeated misconduct, or refusal to follow instructions may result in removal from the camp without refund.
• The camp is not responsible for lost or damaged personal items.
• The camp reserves the right to refuse participation if safety is at risk.
• I agree to the rules and regulations.
