In support of DB InnerFit's commitment to community wellness, all participating vendors are required to contribute a $25 gift certificate to be used at our gym.

Booth Personnel *

You agree to limit the number of personnel at your booth to yourself and two (2) additional representatives.

Table and Equipment *

Please note that you are responsible for bringing your own table, chairs, display materials, and any other equipment you may need for your booth.

Waiver *

By participating in the DB InnerFit Health and Wellness Fair, you agree to hold DB InnerFit harmless from any and all claims, damages, losses, or liabilities arising from or related to your participation in the fair. This includes, but is not limited to, loss, theft, or damage to your property, personal injury, or any other incident that may occur during the event.
