By selecting “I Agree” below, I acknowledge that I have read, understood, and agree to the following:

1. Acknowledgment of Risk I understand that participation in exercise, fitness classes, strength training, and use of equipment involves inherent risks, including but not limited to injury, illness, permanent disability, or death. These risks may arise from my own actions or inactions, the actions of others, the condition of the facility, or the use or malfunction of equipment.

2. Voluntary Participation and Assumption of Risk I voluntarily choose to participate in activities at The Vent: Human Performance Center and knowingly and voluntarily assume all risks, both known and unknown, associated with my participation.

3. Release of Liability To the fullest extent permitted by law, I, on behalf of myself and my heirs, assigns, and personal representatives, hereby waive, release, and discharge The Vent: Human Performance Center and its owners, officers, employees, coaches, contractors, and agents (the “Released Parties”) from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, damage, or loss that may occur as a result of my participation. This includes, but is not limited to: Participation in any class, program, or training session Use of any equipment or facilities Instruction, supervision, or coaching Equipment malfunction or failure Slips, trips, falls, or other accidents on or around the premises This release includes claims based on the active or passive negligence of the Released Parties, to the fullest extent permitted by California law.

4. Health and Medical Responsibility I confirm that I am physically able to participate in exercise activities or have consulted with a medical professional prior to participation. I agree to inform staff of any medical conditions that may affect my ability to safely participate. I understand that I should stop exercising immediately and notify staff if I feel pain, dizziness, lightheadedness, or discomfort.

5. Equipment Use and Personal Responsibility I agree to use all equipment as intended and to follow all instructions and safety guidelines provided by staff. I understand that improper use of equipment may increase the risk of injury.

6. No Guarantee of Supervision I understand that staff may not be present or monitoring at all times, and I accept full responsibility for my participation.

7. Emergency Medical Authorization I authorize The Vent staff to obtain emergency medical care on my behalf if deemed necessary and agree to be responsible for any costs associated with such care.

8. No Guarantee of Results I understand that no specific results are guaranteed from participation in any fitness program.

9. California Civil Code Section 1542 Waiver I expressly waive the protections of California Civil Code Section 1542, which states: “A general release does not extend to claims that the creditor does not know or suspect to exist in his or her favor at the time of executing the release, which if known by him or her would have materially affected his or her settlement with the debtor.”

10. Severability If any portion of this agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect.

11. Electronic Agreement I agree that my electronic acceptance of this agreement is the legal equivalent of my handwritten signature.

12. Acknowledgment and Agreement I certify that: I have read and fully understand this waiver I am agreeing voluntarily I am at least 18 years old, or a parent/legal guardian agreeing on behalf of a minor I understand that this is a waiver of liability and that I am giving up certain legal rights
