I certify that I am in good health and capable of undertaking this tour. I agree to notify the tour operator of any medical conditions or dietary requirements that may affect my participation.

I, the undersigned, release William Henry and any associated individuals or organizations from liability for any accidents, injuries, illness, loss, theft, or other mishap that might occur for any reason in connection with this journey. This includes but is not limited to acts of God, terrorist acts, governmental or political actions, civil unrest, fires, epidemics, quarantine, or failure of any Travel Supplier. I understand that William Henry acts only as an agent for independent Travel Suppliers and is not financially responsible for cancellation of all or part of any tour due to circumstances beyond his control.

I understand that attendance on this tour constitutes acceptance of all terms, conditions, and provisions of the responsibility clause as stated in the Tour Information & Terms document.
