Momentum Pro Sports Participant Activity Waiver

Waiver and Release
IN CONSIDERATION of the acceptance of my application and the permission to participate as a participant or competitor in the Volleyball Activities to be held in 2026, both inclusive, I, for myself, my heirs, executors, administrators, successors and assigns HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE Michael Amoroso Services Inc. O/A Momentum Pro Sports, and all other associations, sanctioning bodies and sponsoring companies, and all their respective agents, officials, servants, contractors, representatives, elected and appointed officials, successors and assigns OF AND FROM ALL claims, demands, damages, costs, expenses, actions and causes of actions, whether in law or equity, in respect of death, injury, loss or damage to my person or property HOWSOEVER CAUSED, arising or to arise by reason of my participation in the said events or activities, whether as spectator, participant, competitor or otherwise, whether prior to, during or subsequent to the event.

I FURTHER HEREBY UNTERTAKE TO HOLD AND SAVE HARMLESS and AGREE TO INDEMNIFY all of the aforesaid from and against any and all liability by any or all of them arising as a result of, or in any way connected with my participation in the said events or activities.

BY SUBMITTING this ENTRY, I ACKNOWLEDGE HAVING READ, UNDERSTOOD AND AGREED to the above RELEASE, WAIVER AND INDEMNITY. I WARRANT that I am physically fit to participate in these events or activities. Do not participate if you have any medical or physical condition preventing participation in these strenuous activities.

I understand that Michael Amoroso Services Inc. assumes no responsibility for my personal safety or loss of personal property. I will obey all safety instructions, established by Michael Amoroso Services Inc. During the course of your activity, photographs may be taken of the spectators & participants.

Rights to Use Images
In participating in these activities, I hereby grant permission to Michael Amoroso Services Inc., the right to use my physical likeness without restriction in any promotion or promotional material created by or for the Michael Amoroso Services Inc. programs or events. I accept that no fee or renumeration will be provided for my appearance in any Michael Amoroso Services Inc. program and/or related promotions and grants unlimited use of my image for this purpose only.
