In consideration of being permitted to participate in any Gellyball activities including, but not limited to, playing, using the premises of, renting and operating equipment leased, sanctioned and/or operated by Northeast Texas Gellyball LLC (NTG), I acknowledge and agree that:
I fully understand and acknowledge that:
Risks and dangers exist in my use of Gellyball equipment and participation in activities.
Participation may result in injury, illness, paralysis, blindness, heat stroke, heart attack, death, or other serious conditions.
These risks may be caused by negligence of NTG owners, employees, agents, participants, others, accidents, breaches of contract, or forces of nature.
I voluntarily assume all risks and responsibilities for losses or damages.
I hereby release, waive, discharge, hold harmless, defend and indemnify Northeast Texas Gellyball LLC (NTG), its owners, agents, officers and employees from any and all claims, actions or losses for bodily injury, property damage, wrongful death, loss of services or otherwise arising from participation in Gellyball activities.
This waiver is valid through 9/19/2026.
If the participant is a minor, the undersigned parent or guardian gives permission for NTG to authorize emergency medical treatment as deemed necessary.
By signing below, I acknowledge that I have read and agree to this waiver and release of liability.
